Updated Return to Activity Form - Covid 19

We have updated our Covid 19 Return to Activity Form, which includes the primary care provider's ability to dictate at what state the student-athlete can return to activity based on their evaluation.  This form will be sent out to each family as new cases are reported, and will need to be returned prior to beginning any team activities.  Stay Safe!
